1. Navigate to the Surveys and Forms component.
2. Click 'Create a form or survey' if the component does not contain any previous forms or surveys.
Alternatively, if the component already contains content, select 'CREATE SURVEY'.
3. Complete the following fields:
- Title
- Open and due dates and times
- Description if required.
- Completion message if required
4. Select from the relevant options
- Participants can submit multiple responses
- Anonymise results - this hides usernames from the results and exported result files
- Notify creator via email when response received
5. Select 'Create' - this will take you to the section creation page.
6. Add a title to the section.
7. Add a description to the section if required.
8. Select 'Create Section'
9. Create a question by selecting a question type, filling in the details and clicking 'Add Question'.
The question types available include:
- Child Selector (allows a parent to identify which child the survey response refers to),
- Date, and
- Date and Time.
You can continue to add questions by selecting question types and clicking 'Add Question'.
Once you have finished, there is no need to save as this is done automatically. Simply, use the breadcrumbs menu at the top to navigate back to your class page, the start of the survey or a particular section.
